Docs: add a worked modal screen-reader regression example?

Would maintainers be open to a small documentation PR adding this free worked example to Modal → Accessibility → Further reading (or to the About modal accessibility page, if that is the better home)?

Why it may fit:

• It reproduces a modal that opens visually but fails screen-reader users because focus remains behind it.
• The walkthrough covers initial focus, focus trapping and restoration, dialog semantics, accessible naming, and background isolation.
• It would complement the existing W3C APG reference, not replace normative PatternFly or W3C guidance.

Proposed change: one Markdown bullet labelled “Worked example: fixing a modal screen-reader failure” in the relevant Further reading section.
